Atopic dermatitis late-breakers from EADV 2026 in Vienna are the focus of this Dermatology Times interview with Christopher Bunick, MD, PhD, associate professor of dermatology at Yale School of Medicine and editor in chief of Dermatology Times. Filmed at Fall Clinical 2026, the discussion covers new late-breaking data and a biomarker analysis comparing upadacitinib and dupilumab. Dr. Bunick reviews atopic dermatitis late-breakers presented at EADV 2026, including tilrekimig, a tri-specific biologic targeting TSLP, IL-4, and IL-13, which showed skin efficacy and itch efficacy. He also highlights an early look at a RAC2 inhibitor in atopic dermatitis. RAC2 is a protein not yet discussed in this disease, and the late-breaker of nine patients reported a dramatic response in EASI score. He then explains upadacitinib vs dupilumab biomarker data from the randomized controlled Level Up trial, including histology from patient biopsies. Dr. Bunick describes more extensive improvement in inflammatory pathways and earlier, more significant improvement in skin barrier genes such as filaggrin and loricrin with the JAK inhibitor. He discusses how a mono-targeted biologic hitting IL-4 receptor alpha compares with selective JAK1 inhibition across IL-4, IL-13, IL-31, TSLP, IL-22, and interferon gamma. Watch for his perspective on which of these late-breaking atopic dermatitis programs to follow.
