In an effort to reduce the number of readmissions that result after a patient leaves the hospital, physicians in Toronto looked at a new model of care called the “Virtual Ward,” to determine whether or not it helped in preventing patients from readmissions. The goal of the new model is to provide a customized care plan which would be specific to each patient. Procedures include personal phone calls, clinic and home visits, and coordinating care with the patient’s primary care physician.
