Partnership to Fight Chronic Disease is a national coalition of patients, providers, community organizations, business and labor groups and health policy experts, committed to raising awareness of policies and practices that save lives and reduce health costs through more effective prevention and management of chronic disease.

Sarah Yourman, an 18-year-old who suffers from cystic fibrosis, diabetes and Crohn’s disease is a compelling speaker and advocate. In her daily life, she must balance school, sports and other typical teenage activities with lengthy lung treatments and multiple medications. She credits quality healthcare, disease management and treatment innovations for what she still believes is a relatively normal teenage life.
